When a system is working, it is very easy to leave it alone. If orders are being processed, reports are being produced, websites are running and teams are getting through the day, reviewing the underlying systems can quickly drop down the list. But over time, small issues can build up. A manual workaround becomes part of the normal process. A spreadsheet becomes the only place where important information is joined together. A website plugin, integration or automation carries on running quietly in the background, even though nobody is completely sure who owns it anymore. None of these things are unusual.
